SOME BACKGROUND

For my service design studio class I redesigned a incident report form.

SMC's incident report form is an essential part of the Santa Monica College's (SMC) risk management strategy. The SMC reporting system ensures that SMC's health, safety & response teams have sufficient information to identify and assess concerns. 

THE SOLUTION

Introduction of new online reporting system to ease reporting and make it easier to analyze, anticipate, and address situations of concern. Upon completing the online report form, students are immediately connected to  appropriate counseling and support resources if needed. 

Research

I employed a variety of methods to address the research and current service questions including document reviews and identification of current services, key informant interviews, focus groups, analysis of existing data, and collection of new data. 

To gain perspective of current safety and support programs, I reviewed publicly available information on the college website. I then conducted interviews with members the 6 different campus team that currently address  health, safety, conduct related concerns. This information collect below in a chart that shows team team's  team's role, process, and available resources.

Stakeholder Map

To develop a comprehensive understanding of the full landscape of SMC's health and safety programs, I constructed an inventory of local and outside community partnerships. This process allowed me to also identify other key stakeholders in the community that directly impact their programs.

Design Direction

After reviewing the research I was able to pinpoint a clear vision for the project. SMC's goal was to strengthen the climate for reporting concerns relating to safety and mental health on campus. However, SMC also wanted to focus on what happens after a student brings a concern to their attention. Reporting and support resources were very separate departments. The new goal was to create a holistic service that balances these two departments.

Goals of Redesign:

1. Greater focus on efforts to report and address issues of concern on campus

2. Introduction of new online reporting system to ease reporting and make it easier to analyze, anticipate, and address situations of concern. 

2. Enhance efforts to connect students with counseling and support resources when needed.
